ABSTRACT

Cachexia is a common complication in patients with lung cancer. It aggravates the toxic and side effects of chemotherapy, hinders the treatment plan, weakens the responsiveness of chemotherapy, reduces the quality of life, increases complications and mortality, and seriously endangers the physical and mental health of patients with lung cancer. The causes and pathogenesis of tumor cachexia are extremely complex, which makes its treatment difficult and complex. Controlling cachexia in lung cancer patients requires many means such as anti-tumor therapy, inhibition of inflammatory response, nutritional support, physical exercise, and relief of symptoms to exert the synergistic effect of multimodal therapy against multiple mechanisms of tumor cachexia. To date, there has been a consensus within the discipline that no single therapy can control the development of cachexia. Some therapies have made some progress, but they need to be implemented in combination with multimodal therapy after fully assessing the individual characteristics of lung cancer patients. This article reviews the application of drug therapy and nutritional support in lung cancer patients, and looks forward to the research direction of cachexia control in lung cancer patients.

ABSTRACT

Objective To investigate the influence of ROI-C cervical implantation in the C5-6 segment on the C3-7 range-of-motion (ROM), biomechanical properties of the intervertebral disc and the vertebral body, and the mode of force transmission. Methods Two types of surgeries, ROI-C implantation and autograft fusion with plate fixation, were considered to establish the finite element model of cervical C5-6 segment degeneration. The ROM of C3-7 during flexion, extension, lateral bending, and axial rotation, as well as stress distributions on the adjacent discs, vertebral body, and implanted devices under two procedures, were analyzed. Results ROI-C implantation had a relatively small influence on the ROM of the adjacent segment. The stress on the vertebral discs was reduced, but the stress on the vertebral body increased significantly, with the C5 vertebral body stress increasing by 251%. In the fusion surgery model, the ROM of the surgical segment was reduced by 86%-91%, while the ROM of the adjacent segments and the stress on the vertebral disc and vertebral body increased significantly. Conclusions ROI-C implantation surgery has a relatively small influence on the cervical ROM and the intervertebral disc, and it has a greater impact on the vertebral body. The research findings provide a theoretical basis for the design of operation plans and clinical studies on ROI-C implantation and autograft fusion with plate fixation.

ABSTRACT

Objective To establish the three-dimensional finite element model of human lower cervical spine C3-7 motion segments after anterior cervical corpectomy and fusion (ACCF) surgery with titanium mesh and bone graft, and to analyze the stability of cervical spine and stress distribution of internal fixation devices after ACCF surgery. Methods The finite element model of cervical spine C3-7 segments after ACCF of C5 segment with titanium mesh, bone graft, plate and screw fixation was established, and C3-7 segment intact model of cervical vertebra was also constructed. The torque moment of 0.5,1.0,1.5,2.0 N · m was applied to the ACCF surgery model. The ROM,maximum stress in facet joint and stress distributions on internal fixation devices under flexion, extension, lateral bending and axial rotation movement were analyzed. Results ROM of reconstructed C5 segment increased with the torque moment increasing after ACCF surgery. In the case of 1.0 N · m tomue moment and 50 N preload, the ROM of reconstructed C5,C3-4,C6-7 and C3-7 segment was reduced by 81%,62%,58% and 80% compared with the intact model. The maximum stress in facet joint of reconstructed C5 segment reduced and the stress in adjacent segments significantly increased. The stress of titanium mesh was mainly distributed on the compression side of movement, and high stress was located in the roots of screws. Conclusions ACCF surgery can promote the stability of cewical spine, decrease the stress in facet joint of operation segment, and has better treatment effect on easing compression from spinal cord caused by cervical spondylotic myelopathy. The research results will provide some theoretical basis for clinical application of ACCF surgery.

ABSTRACT

OBJECTIVE:To provide reference for rational use of parenteral nutrition. METHODS:Combining with the pa-tients'nutritional status, clinical diagnosis and lab examination,5770 parenteral nutrition medical orders were collected from our hospital during May 2015-Apr. 2016,and then analyzed in respects of the stability of drug use, rationality of drug selection and pharmacoeconomics. RESULTS:The qualified stability of nutrient solution in 5770 parenteral nutrition medical orders accounted for 89.46%. The rates of reasonable drug selection in patients with abnormal liver function, renal function, hyperemesis gravidarum,pan-creatitis,complete fasting, long term parenteral nutrition (>14 d) and triacylglycerol >4.5 mmol/L were 76.19%,63.16%, 75.00%,78.95%,65.95%,64.06%and 50.00%,respectively. Four kinds of drugs were with same effect and dose, price difference between 4-34 times, the rate of inappropriate drug selection ranged 7.58%-61.64%. CONCLUSIONS:Parenteral nutrition design need to not only consider about nutritional status, clinical diagnosis and lab examination, but also take into account high efficiency, safety, economy and the stability of whole nutrient solution.

ABSTRACT

This study was aimed to explore recognition models and to establish statistical pattern recognition methods of cold-hot property markers based on lipid components GC-MS chromatogram of Chinese materia medica (CMM). GC-MS fingerprints of lipid components contained in 60 kinds of cold or hot property of CMM were used as the research object. The database was established. Five types of model establishment strategies were compared. Optimal modeling patterns were screened for the identification of herbal property markers of lipid components GC-MS chromatogram. The results showed that support vector machine (SVM) was the best model to discriminate cold or hot property among 60 types of CMM, which were able to effectively mark the characteristic area. The strongest markers tending to cold property was at the retention time of 61.550 min, while the strongest markers tending to hot property was at the retention time of 31.395 min. It was concluded that cold or hot property of CMM had close relationship with lipid components. The lipid component was one of the material bases of CMM. The mathematical statistical model based on material base and herbal property can be used to identify and predict the cold and hot property of CMM.

ABSTRACT

This study was aimed to investigate the seed protein of different forms ofTrichosanthis fructus from Shandong province. UV analyzer was used to analyze the seed protein content. PAGE fingerprint was drawn by Biosens Gel Imaging System software. The Jaccard coefficient was worked out by using NTSYS-pc software. A cluster dendrogram of different samples was established based on unweighted pair-group method with arithmetic means (UPGMA). The results showed that the content of different samples was obviously different. But there were many common bands in the fingerprints of PAGE. It was concluded that the content of seed protein cannot be used as the main index to evaluate the quality ofTrichosanthis fructus. The common bands in fingerprints of PAGE can be used to identifyTrichosanthis fructus.
